How do I make an appointment?
Patients are seen by appointment from 9:00am to 7:00pm,
Monday through Friday. Physical therapy patients are seen
by appointment from 8:00am to 7:30pm, Monday through Friday and Saturday mornings.
Appointments may be scheduled by calling our office at
630-355-3774 from 8:30am to 5:30pm
Will I have to wait before seeing
the doctor?
We do make every effort to keep our appointment schedule
on time, but emergencies and other unforeseen events sometimes
cause unavoidable delays. In such cases, we will do our
best to keep you informed. If you are unable to keep a
scheduled appointment, please contact the office as soon
as possible so that we may try to utilize that time for
the care of other patients.
What if I have an emergency?
If needed, our doctor and physician assistant can be reached
24 hours a day by calling our regular office number 630-355-3774.
You will reach our answering machine that will direct
you to our paging service. Your call will be returned
in a timely manner. We ask however that you make non-emergency
calls during regularly scheduled office hours only.
What if I need a prescription refill?
Prescriptions can be refilled during regular office hours
Monday through Friday between 8:30 am and 5:30 pm at the
regular office number, 630-355-3774. You will be connected
to the Physician Assistant. If she is not available, you
will be connected to her voice mail. She will return your
call the same day. Please be prepared by having your prescription
and pharmacy phone number available. All prescriptions
require physician approval prior to being phoned into
the pharmacy.
How will I know how much my care
is going to cost?
It is not always possible to give you an exact amount
for office and/or surgical treatment. However, we will
give you an estimate for your care whenever possible.
Fees for surgery will include routine follow up care and
dressing changes for 90 days following the date of your
surgery. Fees do not include follow up casting procedures.
As for office visits, the fee for a first time office
visit will be higher than your follow up office visits.
You will be billed separately for x-rays and medical supplies.
You will receive a separate bill from the surgery center/hospital
for their services.
What if I have insurance?
Most insurance companies have deductible amounts and/or
co-payments. We will verify your coverage before your
initial appointment and advise you of your responsibility
as far as deductible and co-pay amounts. If your carrier
does not have a contract with us and you have a PPO, you will be responsible
for the deductible and percentage not allowed by the carrier.
The patient and/or guardian are ultimately responsible
for the payment of the bill. Payment is expected at the
time of service rendered.
What method of payment is accepted
at your facility?
We accept cash, check, traveler’s check and credit
cards (VISA, MasterCard).
